- Print the Oral Health and Hygiene Assessment form linked on this page. You may also pick up a copy of the form in our office.
- Take the Oral Health and Hygiene Assessment form to your dental provider for their signature.
- Bring the completed form to our office. We will then schedule your assessment consultation appointment.
If your child’s orthodontic care in Anchorage, Alaska, is covered by Denali Kid Care, an Orthodontic Referral Oral Health and Hygiene Assessment needs to be completed by your general dentist before scheduling a visit with us.
